New evidence of a medical condition existing at the time of sentencing but not disclosed will not be admitted on appeal where the diagnosis is uncertain, treatment in custody is not shown to be inappropriate, and the symptoms are not disabling in any very significant degree. Voluntary disclosure of otherwise unknown guilt warrants a significant added element of leniency beyond the discount for a guilty plea, but this does not necessarily require a sentence below the median range where the offending is otherwise serious.
