Continuity of symptoms from an admitted physical injury does not, without supporting medical evidence, compel a conclusion that the injury remains substantially physically based. In serious injury applications, where a worker's case is narrowed on appeal to a single body function, the consequences of that specific injury must independently satisfy the 'at least very considerable' threshold, and evidence that does not differentiate between consequences of multiple injuries will be insufficient.
