Benalla Health, a government-funded public hospital operating an aged care facility, was convicted and fined $230,000 for failing to ensure the safety of residents by not maintaining a system for inspection and maintenance of chairs used to transport residents, where a chair's backrest separated causing a 93-year-old resident to fall and sustain injuries from which she subsequently died.
