A single Form 1 document can only attach additional offences to a single principal offence; offences listed on one Form 1 cannot be taken into account across multiple counts on the indictment. When multiple counts of the same offence appear on an indictment, the Form 1 should clearly identify the specific count to which the additional offences are attached, by including a count number, sequence number, or charge number. On re-sentencing, an offender cannot rely on evidence of pre-sentence matters to run a different case from that advanced at the original sentencing hearing.
