3909 Application for arbitration—injury notice and medical evidence
(1)
An application for arbitration filed in the court must be accompanied by—
(a)
if available to the applicant—a copy of the injury notice in relation to the claim; and
(b)
a copy of all available medical evidence the applicant intends to rely on.
(2)
If no injury notice was given in relation to the claim, the application must contain details of why no notice was given.
