14 Form of amendment of authority
An amendment of an authority must—
identify the authorised operation for which the authority is in force; and
state the name, and rank or position, of the person granting the amendment; and
if the application for amendment is made under section 13 (3)—state the name of the applicant; and
state whether the application is a standard amendment application or was an urgent amendment application; and
state the date and time when the amendment of an authority is or was granted; and
describe the amendment having regard to the purposes mentioned in section 13 (3) in relation to which the application was made.
