4A Information included in application to register dog—Act, s 6
An application to register a dog must include the following information:
the applicant’s name, address, email address or other contact information;
any conviction or finding of guilt of the applicant within the last 10 years against an animal welfare offence or an offence against the Act (other than an excluded offence);
if the dog is kept at another address—the address;
the dog’s name;
if the dog is a recognisable breed—the breed;
the colour of the dog;
the unique identification number for any microchip implanted in the dog;
a statement as to whether or not the dog is an assistance animal, dangerous dog, racing greyhound or registered racing greyhound;
if the dog has been de-sexed—a statement to that effect.
