162 Recording medicines supplied during consultations

A prescriber who supplies a medicine during a consultation must make a written record of the following information in the medical records of the person to whom, or animal to which, the consultation related:

(a)

the date the medicine is supplied;

(b)

the medicine’s approved name or brand name;

(c)

the form, strength and quantity of the medicine;

(d)

the directions given to the person for the use of the medicine.
