35 Information for register of authorised practitioners—Act, s 96 (2)

The following details about an authorised practitioner are prescribed:

(a)

the name, business address and telephone number of the practitioner;

(b)

the practitioner’s registration number;

(c)

the day the director‑general authorised the practitioner;

(d)

whether the practitioner is authorised as an authorised coordinating practitioner, authorised consulting practitioner or authorised administering practitioner;

(e)

if the practitioner is a doctor—their area of specialisation;

(f)

if the practitioner is a nurse practitioner or registered nurse—their relevant area of practice;

(g)

any conditions on the practitioner’s authorisation, other than the conditions mentioned in section 34 (a) and (b).
