41 Information for reviewable decision notice—Act, s 135 (1)
The following information is prescribed:
(a)
details about the reviewable decision made by the decision‑maker;
(b)
a statement that an affected person may apply to the ACAT for review of the reviewable decision;
(c)
a statement about how an affected person may apply to the ACAT for review of the reviewable decision.
