9 Information for final request report—Act, s 34 (1) (a)
The following information is prescribed:
(a)
the individual’s name;
(b)
the individual’s date of birth;
(c)
the individual’s home address;
(d)
the day the individual made the final request;
(e)
whether the individual used an interpreter to make the final request;
(f)
a statement that the individual’s final request met the requirements mentioned in the Act, section 32 (2).
