The following information is primary information:

details of the proprietor (including, where the proprietor is a company, its Australian Company Number) of the diagnostic imaging premises or the base for mobile diagnostic imaging equipment (as the case requires);

the business name under which diagnostic imaging procedures are carried out;

the ABN under which diagnostic imaging procedures are carried out:

in the case of diagnostic imaging premises—using diagnostic imaging equipment that is ordinarily located at the premises; or

in the case of a base for mobile diagnostic imaging equipment—using diagnostic imaging equipment ordinarily located at the base when not in use that is not ordinarily located at diagnostic imaging premises;

in the case of diagnostic imaging premises:

the address of the premises; and

a statement identifying the types of diagnostic imaging equipment ordinarily located at the premises;

in the case of a base for mobile diagnostic imaging equipment:

the address of the base; and

the address of the proprietor; and

a statement identifying the type of each piece of diagnostic imaging equipment that is ordinarily located at the base when not in use and is not ordinarily located at diagnostic imaging premises;

details of the legal relationships that give rise to a right to use the equipment.

The regulations may prescribe types of diagnostic imaging equipment for the purposes of this section.
