4 When a healthcare recipient is eligible for registration
A healthcare recipient is eligible for registration if:
(a)
a healthcare identifier has been assigned to the healthcare recipient under paragraph 9(1)(b) of the Healthcare Identifiers Act 2010; and
(b)
the System Operator has collected the following information in relation to the healthcare recipient:
(i)
full name;
(ii)
date of birth;
(iii)
healthcare identifier, Medicare card number or Department of Veterans’ Affairs file number;
(iv)
sex;
(v)
such other information as is prescribed by the regulations.
