28 Contents of application
The application must include the following:
(a)
a copy of the surrogacy arrangement, including the certificates required under sections 21 and 22(3);
(b)
the report from the counsellor required under section 24;
(c)
a certified copy of the birth certificate of the child born under the surrogacy arrangement.
