21.03 Form of application, &c.
Subject to subrule (2), an application for default judgment or summary judgment is to be made in accordance with Part 6.
An application for default judgment need not be served on the other party if that party has not filed a notice of appearance.
A party who applies for an assessment of compensation or other relief to be assessed must also file an affidavit setting out the amount claimed and the particulars, by the use of sub-totals, of how each payment claimed is made up or calculated.
The affidavit must refer to and have annexed all other documents on which the party relies, including those that provide evidence of the amounts claimed and, if applicable, medical reports.
