39E Replacement of licence
The holder of a casino key employee licence or a casino employee licence may apply to the chief executive for a replacement licence if—
the licence has been damaged, destroyed or lost; or
the holder has changed his or her name.
The application must be accompanied by the fee prescribed under a regulation.
The chief executive must consider the application and either grant or refuse to grant the application.
The chief executive may grant the application only if the chief executive is satisfied—
for an application to replace a licence that has been damaged, destroyed or lost—the licence has been damaged, destroyed or lost; or
for an application to replace a licence because of a change of name—the holder of the licence has changed his or her name.
If the chief executive decides to grant the application, the chief executive must immediately give the applicant a replacement licence.
If the chief executive decides to refuse to grant the application, the chief executive must immediately—
give the applicant an information notice for the decision; and
if the chief executive believes the holder of the licence is an employee of a casino operator—give a copy of the notice to the casino operator.
