91N Self-exclusion notice
(1)
A person may give to a casino operator a notice in the approved form (a self-exclusion notice) asking the casino operator to prohibit the person from entering or remaining in the casino.
(2)
The notice must be accompanied by a recent photo of the person.
(3)
If a casino operator operates more than 1 casino, a self-exclusion notice may relate to a stated casino or all casinos operated by the casino operator.
