152B Record of replacement of early childhood teacher or suitably qualified person
The approved provider of a centre-based service must keep a record of an early childhood teacher or a suitably qualified person who is replaced in accordance with regulation 135(1) or (2) that includes the following information—
the full name of the early childhood teacher or the suitably qualified person;
whether the person who is replaced is an early childhood teacher or a suitably qualified person at the service;
the qualification of the person who replaced the early childhood teacher or the suitably qualified person (as the case may be);
the dates on which the early childhood teacher or the suitably qualified person was replaced;
the reason for the early childhood teacher’s or the suitably qualified person’s absence.
