156 Test reports
(1)
The results of the test must be recorded in a test report.
(2)
The test report must be in the approved form, or if there is no approved form, a form acceptable to the regulator, and contain the following information about the test—
(a)
the provision of this regulation under which the testing was conducted;
(b)
the name of the entity that conducted the test;
(c)
the date of the test;
(d)
the date of the report;
(e)
the results of the test, in the form set out in the relevant standard;
(f)
other information required to be included in the report under this part.
