38 Form of licence
(1)
An interactive gambling licence must be in the approved form.
(2)
The approved form must provide for the inclusion of the following particulars—
(a)
the licensed provider’s name;
(b)
the date of issue of the licence;
(c)
the term for which the licence is (subject to this Act) to remain in force;
(d)
the conditions of the licence;
(e)
other particulars prescribed under a regulation.
