9 When person may access voluntary assisted dying
A person may access voluntary assisted dying if—
the person has made a first request; and
the coordinating practitioner for the person has assessed the person as meeting the requirements of a first assessment of the person; and
the consulting practitioner for the person has assessed the person as meeting the requirements of a consulting assessment of the person; and
the person has made a second request; and
the person has made a final request; and
the coordinating practitioner for the person has certified in a final review form that—
the request and assessment process has been completed in accordance with this Act; and
the practitioner is satisfied of each of the matters mentioned in section 46(3)(b); and
the person has made an administration decision; and
the person has appointed a contact person.
