347 Replacement of lost or stolen card
If a person’s working with children card is lost or stolen (the lost or stolen card), the person must, within 14 days after the loss or theft—
give the chief executive a notice about the loss or theft in the approved form and in an approved way; and
either—
apply for a replacement card; or
ask the chief executive under section 304D to cancel the person’s working with children authority.
Maximum penalty—10 penalty units.
An application under subsection (1)(b)(i) must be—
made in the approved form and in an approved way; and
accompanied by the prescribed fee for a replacement card.
The chief executive must—
cancel the lost or stolen card; and
if the person applied for a replacement card under subsection (1)(b)(i)—issue a replacement working with children card to the person.
The chief executive must give written notice about the loss or theft of the lost or stolen card to the police commissioner.
