79 Identification of party
A document is to have the following information shown below a horizontal line drawn at the foot of the first page:
if the party is represented by a practitioner – the name of the party on whose behalf the document is filed; and the name, address, telephone number and facsimile number of the practitioner; and if an agent for the practitioner acts in the proceeding, the name, address, telephone number and facsimile number of the agent; and the name of the practitioner who has carriage of the proceedings in which the document is filed; and the address of the practitioner for the reception of documents in a document exchange; and if the practitioner accepts service of documents by email, the email address of the practitioner;
if the party is not represented by a practitioner – the name, telephone number and facsimile number of the party for whom the document is filed; and the address for service of the party; and if the party accepts service of documents by email, the email address of the party.
