38 Prescribed information—application for service waiver
For the purposes of section 86(b) of the Act, the following information is prescribed for an application under section 85 of the Act for a service waiver—
the name of the children's service;
the service approval number;
the name and contact details of the contact person for the purposes of the application;
a statement that specifies—
the regulations in relation to which a service waiver is sought; and
the way in which the children's service does not or will not comply with the specified regulations;
if the children's service is unable to comply with the specified regulations—
the reasons that the children's service is unable to comply; and
details and evidence of any attempts made to comply with the specified regulations;
the reasons that the children's service seeks the service waiver;
the measures being taken or to be taken to protect the wellbeing of children being educated and cared for by the service while the service waiver is in force.
