16 Application for variation of registration of a health service establishment
(1)
For the purposes of section 92(2)(a) of the Act, the prescribed form is the form in Schedule 6.
(2)
For the purposes of section 92(2)(b) of the Act, the prescribed fee—
(a)
in the case of an application for the transfer of a certificate of registration or certificate of renewal of registration to another person who intends to become the proprietor is 478 fee units; and
(b)
in any other case is 161 fee units.
