20 Form of notice to comply with licence condition
For the purposes of section 37(2)(a) of the Act, the prescribed form is a form that contains the following information—
the name of the licence holder to whom the notice applies;
the provision of the Act or these Regulations (as the case may be) that are reasonably believed to have been contravened;
the grounds for suspension or cancellation (as the case may be) that are reasonably believed to exist;
a statement that requires the licence holder, within the time specified in the notice, to take such steps as are reasonable in the circumstances to remedy the non-compliance or to remove the grounds on which the licence could be suspended or cancelled;
the time within which the notice must be complied;
that the holder of the licence may apply to VCAT for review of the decision to issue the notice.
