17 Particulars for application for relocation of licence or BYO permit
For the purposes of section 31(2)(b) of the Act, the prescribed particulars are—
the applicant's name; and
the applicant's address for service of documents; and
a telephone number on which the applicant or their representative can be contacted during business hours; and
the name and address—
if the applicant is a body corporate—of each director of the body corporate; or
if the applicant is a member of the committee of management of an unincorporated club applying on behalf of the club—of each member of the committee of management of the club; or
if the applicant is a member of a partnership applying on behalf of the partnership—of each member of the partnership; and
the current address of the licensed premises and the address of the premises to which it is proposed that the licence or BYO permit be relocated; and
the licence or permit number.
