6.06 Applications for restraining orders
(1)
An application for a restraining order under the Act (other than a civil forfeiture restraining order or an unexplained wealth restraining order) shall be in Form 6–6A.
(2)
An application under section 36K of the Act for a civil forfeiture restraining order shall be in Form 6–6AB.
(3)
An application under section 40F of the Act for an unexplained wealth restraining order shall be in Form 6–6AC.
