22DI Form and content of application

An application under section 22DH must—
(a)
be made in writing; and
(b)
be supported by an affidavit that—
(i)
sets out the grounds on which the applicant considers that the support and engagement order should be extended; and
(ii)
is sworn or affirmed by the applicant; and
(c)
specify the period for which the applicant wants the order to be extended; and
(d)
include the prescribed information.
