8 Voluntary assisted dying permit
For the purposes of section 49(5) of the Act, the prescribed form is—
(a)
in the case of a self-administration permit—Form 3 in Schedule 1; and
(b)
in the case of a practitioner administration permit—Form 4 in Schedule 1.
