Worker Screening Act 2020

The Regulator must approve the form in which an application for a screening check is to be made.

Without limiting section 15 or 54, an approved application form must include the following particulars—

the full name of the applicant and any other names by which the applicant is or has been known;

the date and place of birth of the applicant;

the gender of the applicant;

the residential address and telephone number of the applicant;

any other information relating to the applicant that the Regulator reasonably believes is appropriate.

The Regulator must publish a copy of an application form approved under this section on a website maintained by the Regulator.
