8 Register: additional information to be recorded (s. 49(2)(d))
For the purposes of section 49(2)(d) of the Act, the following information is prescribed as information to be included in the register in respect of a registered pharmacy —
(a)
the name and business address of each person who owns, or holds a proprietary interest in, the pharmacy business carried on at the premises;
(b)
the nature of the interest held by each person referred to in paragraph (a);
(c)
the name and business address of the pharmacist who has overall responsibility for the pharmacy business carried on at the pharmacy under section 56 or 58(2) of the Act.
