When request and assessment process completed

In force

For the purposes of this Act, the request and assessment process has been completed in respect of a patient if the coordinating practitioner for the patient —

(a)

has completed the final review form in respect of the patient; and

(b)

has certified in the final review form that the request and assessment process has been completed in accordance with this Act.
