40 Form of authorisation
An authorisation must—
(a)
be in writing; and
(b)
state why the authorisation is required; and
(c)
state the full name and address of the person to whom the authorisation is granted; and
(d)
state the licensed premises in relation to which the authorisation is granted; and
(e)
state the period for which the authorisation is granted; and
(f)
state any condition put on the authorisation by the animal ethics committee.
