60 Replacement of licence
The work health and safety commissioner may issue a replacement licence to a licensee if satisfied that the licence has been lost, stolen or destroyed.
For subsection (1), the work health and safety commissioner may require the licensee to give the work health and safety commissioner a statement signed by the licensee, verifying that the licence has been lost, stolen or destroyed.
It is an offence to make a false or misleading statement, give false or misleading information or produce a false or misleading document (see Criminal Code, pt 3.4).
A licence issued under this section must be issued for the remainder of the period of the licence it replaces.
