61 Surrender of licence
(1)
A licensee may apply to the work health and safety commissioner to surrender the licensee’s licence.
(2)
The licensee must, with the application—
(a)
return the licence to the work health and safety commissioner; or
(b)
if the licence has been lost, stolen or destroyed—give the work health and safety commissioner a statement signed by the licensee, verifying that the licence has been lost, stolen or destroyed.
Note It is an offence to make a false or misleading statement, give false or misleading information or produce a false or misleading document (see Criminal Code, pt 3.4).
