18 Form of licence
A licence must—
be in writing; and
include the following information:
the full name and address of the licensee;
a unique identifying number;
the term of the licence;
any conditions on the licence;
any other information prescribed by regulation; and
identify or describe—
each regulated radiation source to which the licence applies (the relevant source); and
each kind of dealing authorised by the licence in relation to each relevant source.
The licence may include any other information the chief health officer considers relevant.
