586 Approved forms—Complaints Commissioner functions
When a document is in the approved form
A document (such as an application, election, request or notice) under this Act that relates to a function of the Complaints Commissioner is in the approved form if:
it is in a form approved by the Complaints Commissioner for that kind of document; and
it contains the information that the form requires, and any further information, statement or document as the Complaints Commissioner requires, whether in the form or otherwise; and
for a kind of document that is required to be given to the Complaints Commissioner—it is given in the manner that the Complaints Commissioner requires (which may include electronically).
Approval of forms
The Complaints Commissioner may, in writing, approve a form for a kind of document under this Act that relates to a function of the Complaints Commissioner.
The Complaints Commissioner may combine in the same approved form more than one kind of document.
The Complaints Commissioner may approve:
more than one approved form for a kind of document; or
a different approved form for a kind of document for different circumstances.
