CERTIFICATE OF CONSCIENTIOUS OBJECTION TO MEMBERSHIP OF ASSOCIATION
I, , *the/*a General Manager or delegate to the General Manager, under subsection 180(1) of the Fair Work (Registered Organisations) Act 2009, certify that:
on application made by (name of applicant) of (address of applicant), I am satisfied that *his/*her conscientious beliefs do not allow *him/*her to be a member of an association of a kind described in paragraph *18(1)(a)/ *18(1)(b)/ *18(1)(c) of the Act; and
(name of applicant) has paid the prescribed fee.
