36.53 Title of appeal books
The title page of each Part of the appeal books must include the following:
(a)
the title of the proceeding;
(b)
the court appealed from;
(c)
the names and addresses for service of the lawyers for each party to the appeal;
(d)
if a party is not represented by a lawyer—the address for service of the party.
