55 Certification by administering practitioner following administration of voluntary assisted dying substance
This section applies if the administering practitioner for the person administers a voluntary assisted dying substance to the person.
The administering practitioner must certify in writing—
that the person made a practitioner administration decision and did not revoke the decision; and
that the administering practitioner was satisfied at the time of administering the voluntary assisted dying substance to the person—
that the person had decision-making capacity in relation to voluntary assisted dying; and
that the person was acting voluntarily and without coercion; and
any other matter prescribed by regulation to be certified.
The certificate must be in the approved form (the practitioner administration form) and must include the certificate of the witness required under section 54.
Within 2 business days after administering the voluntary assisted dying substance, the administering practitioner must give a copy of the practitioner administration form to the board.Maximum penalty—100 penalty units.
