23 Prescribed forms of consent
For the purposes of section 30 of the Act, the prescribed form of consent to the adoption of a child is – Form 5, in the case of consent other than under paragraph (b) or (c); or Form 6, in the case of consent to the adoption of a child by – a spouse of a natural parent, or of an adoptive parent, of the child; or a natural relative of the child; or 2 persons, one of whom is a natural relative of the child; or Form 7, in the case of consent given by the Secretary under section 70 of the Children, Young Persons and Their Families Act 1997 to the adoption of a child who is under the guardianship of the Secretary.
For the purposes of section 32 of the Act, the prescribed form of consent given by the guardian or delegate of the guardian to the adoption of a child who is a non-citizen child is Form 7.
