24 Taking and witnessing of consents
For the purpose of section 30(1)(a)(i) of the Act, an approved counsellor is a prescribed person.
For the purpose of section 30(1)(b) of the Act, the prescribed statement by a witness is contained in Form 5 or 6.
For the purpose of section 30(2)(b) of the Act, the prescribed statement by a witness is contained in Form 5 or 6.
