39 Applications for, and certificates as to, counselling, &c.
For the purposes of section 80 of the Act –
an application to a relevant authority for a certificate to the effect that the requirements of that section have been complied with is to be in accordance with Form 16; and the certificate to the effect that the requirements of that section have been complied with is to be in accordance with Form 17.
