in the case of any architect—

the date of registration; and

the registration number; and

the class of registration; and

if the architect holds a specialist qualification, the nature of that qualification;

in the case of an architect who is a member of an approved partnership or a director of an approved company—

the name and approval number of the approved partnership or approved company; and

when the architect becomes a member or director and, if applicable, ceases to be a member or director;

in the case of an approved partnership—

the date of approval; and

the approval number; and

the name and address of each member of the partnership; and

the name and registration number of each architect member;

in the case of an approved company—

the date of approval; and

the approval number; and

the name and registration number of each architect director;

in the case of a practising architect, the required insurance information.
