14 Information to be included with application by natural person
An applicant who is a natural person must include the following information with the application—
the full name and residential address of the applicant;
the proposed name of the applicant's conveyancing business or the name of the employer or proposed employer of the applicant;
the address of the applicant's principal place of business in Victoria;
if the applicant intends to carry on a conveyancing business at more than one place, the address, and the name of the proposed manager, of each other intended place of that business;
the applicant's Australian Business Number;
if the applicant intends to maintain an Internet site in relation to the applicant's conveyancing business, the address for the Internet site;
if the applicant intends to carry on a conveyancing business in partnership, the names of the proposed partners;
if the applicant intends to employ other licensees in the applicant's conveyancing business, the names of the other licensees;
details of the applicant's competency qualifications and experience referred to in section 12;
details of the professional indemnity insurance (including the level of insurance) held by the applicant or the evidence referred to in section 11(1)(d)(ii);
the prescribed information (if any);
a statement by the applicant to the effect that the information provided with the application is true and correct.
