4.03 Names, addresses and representation of parties
A complaint must—
state the full name and address of the plaintiff and an address for service of documents on the plaintiff; and
if the plaintiff sues in person, state an address for service of documents on the plaintiff within Victoria in accordance with Rule 6.06; and
if the plaintiff sues or the defendant is sued in a representative capacity, state the capacity in which the plaintiff sues or the defendant is sued in a representative capacity; and
state the name and address of the defendant; and
if the plaintiff sues by an Australian lawyer, state the name or firm and business address of the Australian lawyer and also, if the Australian lawyer is the agent of another, the name or firm and business address of the principal.
