38 Incident recording, investigation and reviewing
The safety management system must specify—
(a)
how reportable and non-reportable safety incidents are to be recorded and investigated; and
(b)
the management systems to be used for reviewing and taking action on information that is recorded or identified by an investigation to improve the safety of the pipeline operation.
