22DB Form and content of application
An application under section 22DA must—
(a)
be made in writing; and
(b)
be supported by an affidavit that—
(i)
sets out the grounds on which the applicant considers that the order or plan should be varied; and
(ii)
is sworn or affirmed by the applicant; and
(c)
include the prescribed information.
