30 Recording and notification of outcome of consulting assessment
(1)
The consulting medical practitioner must—
(a)
notify the person requesting access to voluntary assisted dying of the outcome of the consulting assessment; and
(b)
complete the consulting assessment report form and give a copy of it to—
(i)
the Board; and
(ii)
the co-ordinating medical practitioner for the person.
(2)
The consulting medical practitioner must give the copy of the consulting assessment report form to the Board within 7 days after completing the consulting assessment.
